OET for ECFMG Certification: Score Requirements, Pathways & 2027 Deadlines

The Educational Commission for Foreign Medical Graduates (ECFMG) certifies international medical graduates who wish to enter ACGME-accredited residency or fellowship programmes in the United States. ECFMG certification is also required to sit USMLE Step 3 and to obtain a medical licence in most US states.

The USMLE Step 2 Clinical Skills (Step 2 CS) examination — which previously assessed communication and English language competency — was permanently discontinued in 2021. ECFMG replaced it with a Pathways system that includes a mandatory English language proficiency requirement. ECFMG chose OET Medicine specifically because it is designed for healthcare professionals: every scenario in the test — consultations, referral letters, ward handovers — reflects real clinical practice, making it a more valid measure of the communication skills IMGs will actually use in a US residency.

All four sub-tests must reach their minimums in a single test administration. You cannot combine scores from different test dates.

Note that Writing has a lower threshold (300 / C+) than the other three sub-tests (350 / B). This is consistent with ECFMG's recognition that professional writing in English is a distinct skill that develops with practice.

If you fail any single sub-test, you must retake all four and achieve the minimum on all of them in that new sitting. There is no banking of individual sub-test scores between administrations.

ECFMG specifies a minimum date for when your OET Medicine test must have been taken. Scores from tests taken before that date are not accepted.

Plan your test date to leave enough time for results to reach ECFMG. OET releases results approximately 10 business days after your test date, and ECFMG requires an additional 3 business days to process the score update in your Pathways application. Cutting it close to the January 31 deadline is a significant risk — schedule your test no later than early December of the relevant year.

ECFMG currently operates six Pathways. All require OET Medicine. Your eligibility depends on your licensure status and your medical school's accreditation.

If you are unsure which Pathway applies to you, the ECFMG Pathways tool at ecfmg.org walks you through an eligibility check. ECFMG does not allow you to choose a "lower effort" Pathway if you qualify for a more demanding one — Pathway 1 is mandatory for eligible IMGs.

Score submission is largely automatic if you set it up correctly at the time of booking:

• At registration: When booking your OET Medicine test, enter your MyIntealth ID (your ECFMG/MyIntealth account number) or your USMLE ID in the designated field. This links your booking to your ECFMG application and scores are transmitted electronically when released.
• If you forgot to link your ID: Log into your OET candidate account after results are released and use OET's score verification/release process to manually send scores to ECFMG.
• Processing time: After OET releases results (~10 business days post-test), allow a further 3 business days for the score to appear in your Pathways application dashboard.

Always verify your score appears in your MyIntealth account before the January 31 deadline — do not assume it has been received.

ECFMG does not accept IELTS or TOEFL as alternatives — OET Medicine is the only test accepted for the English language proficiency component of the Pathways. ECFMG's rationale is straightforward:

• Clinical specificity: Every OET task uses medical scenarios. Listening extracts are doctor–patient consultations and ward handovers. The Writing task is a referral or discharge letter. The Speaking role-plays simulate real patient interactions. IELTS and TOEFL use general English contexts that do not predict clinical communication ability.
• Profession-specific scoring: The OET Writing criterion "Genre and Style" specifically assesses clinical register — the formal, precise language of professional medical correspondence.
• Single-administration rule: Because all four sub-tests must be passed in the same sitting, a single OET session is a more demanding and complete assessment of functional English than four separate general-English component scores.

The full OET Medicine test takes approximately 3 hours and covers all four sub-tests in a single session.

Delivery options:

• Computer-based — available at test centres worldwide; recommended for candidates comfortable typing.
• Paper-based — handwritten; available at selected centres.
• OET@Home — remote proctored; available only in countries without a physical test centre.

The test fee is approximately AUD 587 / USD 455, though prices vary slightly by country. Retakes have no limit — you can sit OET as many times as needed, but each attempt requires a new registration and full fee.

Most IMGs targeting ECFMG Pathways underestimate OET preparation time. A typical candidate who is a confident English speaker still needs 6–10 weeks of focused preparation to reliably hit 350/350/350/300 in a single sitting — primarily because the test format is unfamiliar, not because the English level is beyond reach.

Where most marks are lost:

• Listening Part A: Note completion under time pressure while processing clinical vocabulary — requires practising writing while listening, not just comprehension.
• Writing: Converting case notes into a formal professional letter is a learned skill. Most candidates default to note-form sentences or include irrelevant content from the case notes, which costs marks on Conciseness.
• Speaking: The role-play format — you play the health professional, not the patient — is unlike any standard conversation exam. Tone, empathy and clinical questioning structure all count.

A realistic 8-week plan:

• Weeks 1–2: Learn the format of each sub-test. Take one diagnostic full-length test under exam conditions to identify your weakest parts.
• Weeks 3–5: Intensive practice on your two weakest sub-tests. For Listening A: daily note-completion drills. For Writing: one timed letter every 2–3 days with criterion-level feedback.
• Weeks 6–7: Full-length practice tests weekly. For Speaking: record your role-plays and review tone and structure.
• Week 8: Review errors, confirm test date and ECFMG ID linkage.
